Medical Documents Translations

With our Medical Document Translation Services, we help your health care facility effectively and efficiently communicate in English with your patients via any written medical document. This service removes  a major barrier to rendering quality health services to Limited English Proficient (LEP) individuals you serve.

This can result in LEP persons not always having equal access to health care or experiencing similar outcomes as English proficient individuals.

As you know, effective communication between you (the provider) and your patient is very critical, and in some instances, can be a matter of life or death.

The Civil Rights Act of 1964 and the Translation Connection

For organizations receiving federal funds, failure to ensure that LEP patients effectively participate in, and benefit from health care services may violate Title VI of the Civil Rights Act (CRA) of 1964. This of course is the act that prohibits discrimination based on race, color, or national origin (among other factors).

Furthermore, the Office for Civil Rights Civil Rights ( a unit within the Department of Health and Human Services) has categorized six (6) types of documents as “vital” documents that must be translated for the purpose of advancing effective communication with LEP language groups. The criteria further specifies that these language groups must constitute either 5% or 1,000 persons (whichever is less) of the total population served. These vital documents are:

– All notices announcing the ability for the LEP to gain free language assistance

– Any notice that specifies the eligibility criteria for receiving services

– All informed consent forms required to be completed by the LEP

– Any and all intake forms that have clinical consequences for the LEP

– All discharge instructions for the LEP

– All complaint forms that can be used by the LEP to provide feedback.

Cesco Linguistic Services will assist in providing comprehensive language access service to the LEP populations you server both interpreting (i.e. oral) and translation (written) services. This includes all of the six “vital” forms and/or any other medical document you need translated
